Latin translation needed

A friend of mine at the local FHC asked for my help in translating a document. It is from a country that uses the cyrillic [sp?] alphabet, but the printed part is also in Latin, as well as what the priest hand-wrote on the form.

Could someone translate the following:

Anno domini millesimo = year of our lord ?

Parenties et conditio = parents and ?

Patrini et conditio = godparents and ?

Ex parte officiii parochialis r. gr. cath = ?

notum testatumque fit, in libris, metricalibus hujus Ecdesiae tomus ____ pag. _____reperiri sequentia = ?

Quas testimoniales manu propia subscrib e et algillo [?] Ecclasiae parochialis corroboro = ?

In the "tomus ____ pag.___" above, it means book and page, but I can't figure out the rest.
In the last phrase above, the priest usually signs off saying that he signs his hand to the testimony, etc., but I don't remember the exact words.
